Thanks to funding through the Toronto Central Local Health Integration Network (LHIN), Madison provides comprehensive, recovery-based case management services to more than 350 clients with complex mental health and/or addictions challenges. Since Madison serves a wide range of individuals from diverse backgrounds, we have honed our skills and developed expertise to help meet particular gaps in the mental health system. For example, we provide services to newcomers, to Spanish-speaking people and to clients who require a high level of support.
Case managers can provide assistance for a wide range of needs including housing, interpretation, medical appointment, day to day activities and access to resources such as counseling.
