Since 1977, Madison Community Services has been providing evidence based supportive housing, congregate care home. We now provide a total of 188 units to our client-tenants, in addition, we provide high intensive needs supportive housing that have engagement and support workers 24/7. The housing programs support clients with their individual needs, three nutritious meals prepared on site, mental health case management and others supports in collaboration with partner.
Madison Community Services manages tenant relations by prioritizing a Housing First approach and eviction prevention strategy by providing stable, permanent housing as the foundation for individuals to improve their overall Mental Health and well-being. This approach is non-judgmental, client-centered, and focused on meeting clients’ needs with respect and dignity. Madison Community Services works closely with clients to ensure that they have access to necessary support services, such as mental health care, and life skills training, regular check-ins with clients, mediation to resolve conflicts, and flexible rent payment plans.
Madison supportive housing in three different categories
- Pathway to Home
The housing program offers of 24/7 intensive on-site high support housing for long-term users of the shelter system who have complex mental health and/or intellectual and developmental disabilities as well as physical health and other challenges. Over 96% of clients enrolled in our program have been able to be successful and maintain their housing after long periods of homelessness.
- Congregagete housing
Madison maintained a portfolio of 7 independent congregate living houses, which house almost 50 clients. Madison focuses on societal integration by providing services 9-5 pm. Clients cook for themselves and meet their own activities of daily living with minimal supervision and are integrated in the communities.
- Partners housing
Madison provides housing in collaboration with its partners like Houseling and Mainstay, St. Claires, TCH, CAMH and Habitat for clients who have been homeless and in the homeless shelters for protracted period across GTA.
- Private landlord
Madison works with private landlord across the Toronto to offer bachelors, one-bedroom and two-bedroom units to clients who require medium-high support and prefer to live independently
